Category Box 2: Indicate what your 2nd category entry choice is from the list or choose "NONE" if you are only entering one category.

​ Category Box 3: Indicate what your 3rd category entry choice is (if you have chosen to enter 3 categories) from the list or choose "NONE" if you are only entering one or two categories.
